To register for Kindergarten for the 2024-2025 school year, children must be 5 years old on or before December 31, 2024.
Please print and complete the Registration form in full, sign in the appropriate areas, and ensure you have completed and attached all the necessary documentation.
Sibling Information form - if you already have a child attending school in the Lord Selkirk School Division, please complete this form.
Medical Questionnaire form - if a medical concern is identified on the registration form, please complete the medical questionnaire.
Authorization for Administration of Prescribed Medication
Bussing is provided to eligible students in the Lord Selkirk School Division. If bussing is required, please complete the Request for Transportation form. If a bus is not required, please indicate that on the registration form.
School of Choice forms - should be completed ONLY if applying to attend a school out of the home school catchment area.
Child in Care form - should be completed ONLY if child is in the care of Child and Family Services.
As outlined by the Department of Records and LSSD, a copy of the child's Birth Certificate, Proof of Citizenship and parent/legal guardian Proof of Residency must accompany all registrations. Acceptable Proof of residency document: Government issued photo identification that includes current address. Manitoba Health is not accepted as proof of residency. To avoid delays in processing, please forward all required documents with the registration.
